Online registration is the most convenient and easiest way to register with Progenics. Unlike most cord blood banks, Progenics has the most sophisticated online registration program, which corresponds to hard copies of our registration forms.  Therefore, there is no extra information required to meet the standards set by AABB (American Association of Blood Banks) and related regulation bodies when the registration is finished.

INSTRUCTIONS:

1. For a new registration, enter your email address and password (case sensitive) and click on “Start a new registration”
2. You will have up to 30 days to complete the online registration
3. For resuming registration, enter your email address and password (case sensitive) and click on “Resume unfinished registration”
4. If you forget your password when you try to resume your registration, click on the “Forget your password?” link and a new password will be emailed to your email account immediately
5. Review the correctness of the information in the summary report before you pay
6. Complete the billing and credit card information
7. Click Continue to pay (A receipt will be emailed to you automatically)
8. Follow the instructions to print the forms:
   • Cord Blood Banking Consent and Agreement
   • Release of Medical Information
   • Pre-Authorized Payment Form (if Pre-Authorized payment is chosen)
   • Summary Report
9. Sign all forms
10. Fax or mail them to Progenics for Progenics' records and for obtaining your medical history from your physician
11. Keep a copy of all forms for your records

Progenics will send you a collection kit with the instructions for clients within 7 business days.
