Register by Fax or Mail

The following information will help you to register by fax or mail or in person.

1. Obtain the registration package

• Download and print from the Progenics website www.progenicscryobank.com
• Click on [Register now] on the menu bar located at the top of the home page.
• Click Download under [Register By Fax and Mail].
• If you are in the vicinity of Leslie and Sheppard (North York, Ontario), feel free to pick up a registration package from our office.
• Simply call our office at (416) 221-1666 or send an email to info@progenicscryobank.com to request a registration package to be mailed to you via Canada Post.

2. Complete the registration forms

• Take the time to read through the registration package which consists of 6 sections (A - F). If you are uncertain about the answers to some questions in Section B: Confidential Health History, leave them unchecked. Our registered nurses will call you to assist you in completing the unanswered questions when we receive the registration package.
• Simply bring in person, mail or fax the completed registration forms to the Progenics office.

• If you have questions regarding the registration package or cord blood banking, and would prefer a (free) one-on-one consultation session with a counselor, do not hesitate to call our office to book a consultation appointment.

5. Obtain cord blood collection kit

If you register in person you will receive a cord blood collection kit upon completion of your registration with Progenics. If you register online or by fax or mail, a cord blood collection kit will be shipped to you at no extra charge once the completed forms and payment are received and accepted (some conditions may apply).
