   3rd due 3 months after the birth of the child

Please note:

• Fees with an asterisk are essential for registration, processing and testing.
• Subsequent installments can be paid by post dated cheque(s) ($20 will be charged for non-negotiable payment eg. NSF) or by credit card but you need to complete a credit card pre-authorized payment form and fax or mail it to Progenics. 
• GST (5%) will apply except on the testing fees.
• Hospitals/physicians may charge a fee for cord blood collection and/or administration.
• Fees subject to change without notice.
